All of our new patients and some return patients are required to fill out paperwork, depending on the type of visit they are seeking. To save time on your appointment, ask what forms you will need for your visit when you make your appointment. Then print them, fill them out, and bring them with you to your visit. Guaranteed to save you at least 15 minutes!
* Our walk-in services, including birth control pick-ups and EC purchases do not require you to fill out these forms.
